Job summary

Powerback Rehabilitation To You is seeking a Speech Language Pathologist to provide in-home outpatient rehabilitation for older adults, in patients' homes or in assisted living facilities. You will evaluate and treat communication and swallowing disorders, promoting functional communication in the home environment.

Responsibilities include developing personalized plans, coordinating with the rehab team, and ensuring timely documentation.

Qualifications

• Must hold a valid Speech Language Pathologist license in the state(s) of practice or have license eligibility.
• Must have a valid driver's license or reliable transportation to patient residences.

Responsibilities

• Provide outpatient rehabilitation services in in-home settings for older adults.
• Assess and treat communication and swallowing disorders in the home and in facilities.
• Coordinate with interdisciplinary team and caregivers to design and progress treatment plans.
• Document patient progress and ensure timely clinical and billing records.
