Job summary

Powerback Rehabilitation to You delivers outpatient rehabilitation in the homes of older adults, or in assisted living facilities and independent living facilities. The Speech Language Pathologist evaluates and treats in the most functional environment, guiding patients to restore, reinforce, and enhance communication and swallowing while adhering to professional ethics.

The role offers a flexible schedule across home and community settings, supervision of CFYs and students where applicable,

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Evaluate and treat in the patient's home or in facilities to facilitate rehabilitation.
• Direct patient participation to restore, reinforce and enhance communication and swallowing.
• Coordinate care with interdisciplinary team and physician to select appropriate therapy.
• Maintain documentation and meet scheduling and billing requirements.
• Supervise CFYs, students and support personnel where applicable according to licensure requirements.
