Job summary

Powerback is seeking a Speech Language Pathologist to deliver outpatient rehabilitation services in an in-home setting for older adults, including private residences or facilities. You will assess communication and swallowing disorders, provide therapy, and collaborate with a multidisciplinary team to optimize patient outcomes.

The role offers a flexible schedule in diverse home environments, with CMEs via Medbridge and pathways for professional growth.

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Supervise CFYs, temporary licensees, students and support personnel in accordance with licensure requirements.
• Adhere to department and patient treatment schedules, complete clinical and billing documentation.
• Attend and contribute to patient care conferences and related meetings.
• Coordinate with other rehabilitation team members and physicians to select appropriate therapies.
• Adapt plans of care based on patient needs and progress.
• Promote safety and maintain a clean environment; ensure equipment is maintained.
