Job summary

Powerback Rehabilitation seeks a Speech Language Pathologist to deliver outpatient therapy in home and facility settings for older adults. The role emphasizes functional, patient-centered care, evaluation and treatment of communication and swallowing disorders, and collaboration with the rehab team to optimize outcomes.

The position involves supervising licensed CFYs and students, maintaining documentation, and adapting plans of care as patients progress, all while ensuring safety and quality

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in home or facility settings for older adults.
• Evaluate and treat in the most functional environment, directing patient participation to restore and enhance performance.
• Facilitate learning of communication and swallowing skills, and document all related services and progress.
• Coordinate with rehabilitation team and physicians to select appropriate therapy plans.
• Supervise CFYs, students and support personnel as required by licensure and standards.
• Adhere to schedules, complete timely documentation, and participate in care conferences.
• Refer to other disciplines to address comprehensive patient needs.
• Adapt plans of care according to patient progress and safety considerations.
• Order supplies and equipment as necessary and maintain equipment.
• Ensure safe environment and eliminate hazards.
• Collaborate with home and community teams for integrated care.
• Provide case management in home/community settings.
• Instruct patients in the use of adaptive communication devices when needed.
• Identify hearing deficits and refer for assessment as appropriate.
• Select activities suited to patient capacity and interests.
• Document patient progress and adjust treatment plans accordingly.
• Maintain high-quality service with compassionate patient-centered care.
• Perform related duties as required.
