Job Title: Patient Care Coordinator

We are seeking a dedicated and compassionate Patient Care Coordinator to join our dynamic healthcare team. Key Responsibilities
  • Serve as a liaison between patients, healthcare providers, and support staff to ensure effective communication and continuity of care.
  • Coordinate patient appointments, follow-ups, and referrals, ensuring timely and accurate scheduling.
  • Assist patients and their families in navigating healthcare services, providing information and support throughout the treatment process.
  • Maintain up-to-date patient records, ensuring accuracy and confidentiality in compliance with healthcare regulations.
  • Work collaboratively with healthcare providers to develop personalized care plans that address patients' needs and preferences.
  • Monitor and evaluate patient progress, making adjustments to care plans as necessary in consultation with the healthcare team.
  • Facilitate communication among multidisciplinary teams to ensure comprehensive care delivery.
  • Provide education and resources to patients and families about diagnosis, treatment options, and post-care support.
  • Address and resolve any patient concerns or complaints promptly, escalating issues to the appropriate personnel when required.

Local travel may be required. Reliable transportation is required.

Qualifications
  • High school diploma or equivalent; additional education in healthcare administration, nursing, or a related field is a plus.
  • Previous experience in a healthcare setting, preferably in patient care coordination or a similar role.
