Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in members’ homes across the U.S. Each visit lasts up to one hour, evaluating medical history, medications, adherence, and social factors to identify care gaps.

Providers order tests, collaborate with PCPs and care teams, and document findings using Matrix protocols. Travel to homes or SNFs is required where needed, with emphasis on clinical excellence and empathetic care.

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• (Board certifications) in Adult/Adult Gerontology/Family/ Emergency specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatments, and referrals; document per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and other healthcare personnel.
• Coordinate with PCP on patient education and follow-up; provide follow-up as necessary.
• Provide services at the member’s residence or in SNFs when applicable.
