Job summary

West Coast Wound is seeking a skilled Physical Therapist to provide high-quality, home-based physical therapy across the Jacksonville region. This 1099 independent contractor role involves evaluating patients in their homes, developing individualized plans of care, and delivering targeted interventions to improve mobility and safety.

The ideal candidate travels to patients' homes, demonstrates strong clinical judgment, and communicates clearly with patients and caregivers.

Qualifications

• Current and valid Florida Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.
• Ability to independently perform comprehensive PT evaluations and develop appropriate plans of care.
• Experience treating orthopedic, musculoskeletal, neurological, balance, gait, mobility, and functional limitations preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and travel throughout the service area.

Responsibilities

• Travel to patients' homes to provide physical therapy services.
• Perform comprehensive home evaluations and create plans of care.
• Provide interventions: exercises, gait training, balance, neuromuscular education.
• Monitor progress and adjust treatment plans.
• Educate patients and caregivers on mobility, safety, and fall prevention.
• Document evaluations, treatment, reassessments, progress, and discharge notes.
• Coordinate with physicians, nurses, caregivers, and other health professionals.
