Job summary

West Coast Wound is seeking a skilled Physical Therapist to deliver home-based services across the S. Los Angeles region. You will perform in-home evaluations, create individualized plans of care, and provide therapeutic interventions tailored to each patient’s goals.

The role is a 1099 Independent Contractor with a 4/10 schedule, four 10-hour days per week, starting around 7:30 AM. Applicants must hold a California PT license and possess strong clinical and communication skills.

Qualifications

• California Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong clinical reasoning and patient assessment skills.
• Ability to independently perform comprehensive PT evaluations.
• Experience with orthopedic, musculoskeletal or neurological conditions preferred.
• Excellent communication and organizational skills.

Responsibilities

• Travel to patients' homes to provide physical therapy services.
• Perform comprehensive home evaluations and develop plans of care.
• Provide skilled PT interventions including exercises and gait training.
• Educate patients and caregivers on mobility, safety, and home exercises.
• Maintain accurate documentation and regulatory compliance.
