Job summary

West Coast Wound & Skin Care is seeking a skilled Physical Therapist to provide home-based PT services across the San Diego region. You will evaluate patients in their homes, develop individualized plans of care, and deliver interventions to improve mobility and safety.

The ideal candidate works independently, communicates clearly with caregivers and providers, and is committed to patient-centered care. This is a 1099 independent contractor role with a four-day workweek.

Qualifications

• California license in good standing as a Physical Therapist.
• Graduate of an accredited Physical Therapy program.
• Strong clinical reasoning and patient assessment skills.
• Ability to develop and implement plans of care independently.
• Experience with home health or mobile therapy preferred.

Responsibilities

• Travel to patients' homes to provide PT services.
• Perform comprehensive home evaluations and develop care plans.
• Provide therapeutic exercises, gait and mobility training, and fall prevention.
• Instruct patients in home exercise programs and safety strategies.
• Document evaluations, treatments, progress, and discharge summaries.
• Coordinate care with physicians and other care providers.
