Job summary

West Coast Wound & Skin Care is seeking a skilled Physical Therapist Assistant to provide home-based physical therapy in the Jacksonville region. You will work under a licensed Physical Therapist to implement plans of care, monitor progress, and educate patients and caregivers for safe, independent functioning.

This full-time 1099 contractor role requires travel to patients' homes, strong communication, and the ability to adapt therapies to varied home environments while maintaining

Qualifications

• Current and valid Physical Therapist Assistant license in good standing.
• Graduate of an accredited Physical Therapist Assistant program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, rehabilitation principles.
• Ability to safely implement a Physical Therapist's established plan of care.

Responsibilities

• Travel to patients' homes to provide PT treatment under supervision.
• Review the plan of care and goals prior to treatment.
• Provide skilled interventions: therapeutic exercises, gait training, balance training, and manual therapy where appropriate.
• Instruct and reinforce home exercise programs for patients and caregivers.
• Document treatment notes and communicate progress to the supervising therapist.
