West Coast Wound & Skin Care seeks a skilled Physical Therapist to deliver in-home PT throughout the Jacksonville region. The PT will perform evaluations, create individualized plans of care, provide interventions, monitor progress, and educate patients and caregivers on mobility, safety, and independence.

The ideal candidate works independently in a home-based setting with strong clinical judgment and communication, ensuring compassionate, patient-centered care.

Formación

• Florida Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Ability to independently perform comprehensive PT evaluations and develop plans of care.
• Experience with orthopedic, neuro, balance, gait, and functional mobility issues preferred.

Responsabilidades

• Travel to patients' homes to provide physical therapy services.
• Perform comprehensive home evaluations and PT assessments.
• Develop individualized plans of care with measurable goals.
• Provide skilled PT interventions: exercises, gait training, balance, functional mobility.
• Educate patients and caregivers on exercises, safety, fall prevention, and independence.
• Document evaluations, progress notes, and discharge summaries.
• Coordinate care with physicians, nurses, and other health professionals.
• Maintain compliance with regulations and professional standards.
