Job summary

West Coast Wound is seeking a dedicated Physical Therapist to deliver in-home therapy across the San Fernando region. You will conduct comprehensive evaluations, craft individualized plans of care, and provide skilled interventions to improve mobility, safety, and independence for patients in their homes.

This 1099 independent contractor role requires strong clinical judgment, excellent communication, and the ability to work autonomously in varied home environments.

Qualifications

• Current and valid California Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics.
• Ability to independently perform comprehensive PT evaluations and develop plans of care.
• Experience with orthopedic, musculoskeletal, neurological, balance, gait or geriatric patients preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Travel to patients' homes to provide physical therapy services.
• Perform comprehensive physical therapy evaluations in the home.
• Assess movement, strength, balance, gait, mobility, and daily living activities.
• Develop individualized plans of care based on evaluation findings and goals.
• Provide skilled interventions including exercises, gait training, balance training, and manual therapy as appropriate.
• Educate patients and caregivers on exercises and safety to promote independence.
