West Coast Wound & Skin Care in Austin, TX is seeking a licensed Physical Therapist to provide mobile home-based care. You will evaluate patients in their homes, develop individualized plans of care, and deliver skilled PT to improve mobility, safety, and independence.

The ideal candidate holds a Texas PT license, has strong clinical judgment and communication skills, and is comfortable working independently.

Qualifications

• Current and valid Texas Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehab principles.
• Ability to independently perform comprehensive PT evaluations and develop plans of care.

Responsabilités

• Travel to patients' homes to provide physical therapy services.
• Perform in-home evaluations and develop individualized plans of care.
• Provide skilled therapeutic interventions and monitor progress.
• Educate patients and caregivers on exercises, mobility and safety.
• Maintain accurate documentation for evaluations, treatments, and discharge summaries.
• Coordinate care with other healthcare professionals.
• Ensure compliance with regulations and company policies.
