West Coast Wound is seeking a skilled Physical Therapist Assistant to provide home-based physical therapy in the Jacksonville region. You will work under a licensed Physical Therapist to implement plans of care, monitor progress, and educate patients and caregivers for safe, independent mobility.

This 1099 independent contractor role offers a 4/10 schedule with four 10-hour days, travel to patients' homes, and comprehensive documentation while maintaining professional standards under Florida PTA

Qualifications

• Licensed Physical Therapist Assistant in Florida with current credentials.
• Experience providing home-based physical therapy is preferred.
• Ability to work independently within PT scope and communicate with supervising PT.

Responsabilités

• Travel to patients' homes to administer PT under supervising therapist.
• Review plan of care and safety precautions prior to treatment.
• Deliver therapeutic exercises, gait training, balance training, and functional strengthening.
• Educate patients and caregivers on home exercise programs and safety.
• Maintain accurate daily treatment notes and documentation.
• Coordinate care with other health professionals.
