Job summary

West Coast Wound is seeking a skilled Physical Therapist to deliver home-based PT across the San Diego region. You will conduct in-home evaluations, develop plans of care, and provide interventions aimed at improving mobility, safety, and independence.

The role is a 1099 independent contractor with a four-day, ten-hour schedule and guaranteed weekly base pay plus per-visit compensation. Travel to patient homes and thorough documentation are essential to success.

Qualifications

• Current and valid California Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.
• Strong clinical reasoning and patient assessment skills.
• Ability to independently perform comprehensive PT evaluations and develop appropriate plans of care.
• Experience treating orthopedic, musculoskeletal, neurological, balance, gait, weakness, mobility, or functional limitations preferred.
• CPR/BLS certification preferred or required according to company policy.
• Valid driver's license and reliable transportation.

Responsibilities

• Travel to patients' homes throughout the assigned service area to provide physical therapy services.
• Perform comprehensive physical therapy evaluations in the patient's home environment.
• Assess strength, range of motion, balance, gait, mobility, coordination, endurance, pain, functional limitations, and ability to safely perform activities of daily living.
• Develop individualized plans of care based on evaluation findings and goals.
• Provide skilled therapeutic interventions including exercises, activities, gait/balance training, and neuroreeducation.
• Instruct patients and caregivers on home exercise programs and safety.
• Maintain documentation, coordinate with other health professionals, and ensure compliance with regulations.
