Job summary

Twin Cities Physicians is seeking a Nurse Practitioner to provide advanced wound care and community-based rounding inside senior living communities in the Twin Cities Metro area. This Senior Living role focuses on AGNP/CNP practice with a strong emphasis on wound management and post-acute care.

The position is full-time with a competitive compensation package including base salary plus productivity incentives, mileage reimbursement, CME allowance, malpractice coverage, and DEA reimbursement.

Qualifications

• Active Minnesota CNP license required.
• Experience in wound care, geriatrics, primary or post-acute care preferred.
• Strong clinical judgment and ability to work independently.
• Valid driver's license and ability to travel throughout Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care services and community-based rounding in senior living communities.
• Manage pressure injuries, diabetic wounds, post-surgical wounds, skin integrity concerns, and complex cases.
• Support chronic disease management, preventative care, and care coordination efforts.
• Collaborate with physicians, nurses, therapists, caregivers, and facility teams.
• Help improve outcomes while reducing avoidable hospitalizations and complications.
• Serve as a trusted clinical partner within the communities supported.
