Job summary

Twin Cities Physicians is seeking a Geriatric Wound Care Nurse Practitioner (AGNP/CNP) to provide on-site wound care assessments and primary rounding across assisted living communities, memory care, and skilled nursing facilities in the Twin Cities Metro.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license.
• Experience in wound care, geriatrics, primary care, post-acute care, or related settings preferred.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
• Strong clinical judgment and ability to manage medically complex patient populations.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Deliver on-site primary care and rounding services throughout senior living communities.
• Manage pressure injuries, diabetic wounds, post-surgical wounds, and complex chronic conditions.
• Coordinate care with physicians, nursing teams, therapists, caregivers, and facility leadership.
• Support preventative care initiatives and continuity of care.
• Help improve healing outcomes while reducing avoidable hospitalizations and complications.
