Job summary

Twin Cities Physicians seeks a Geriatric Wound Care Nurse Practitioner (AGNP/CNP) to provide on-site wound care and primary care in senior living communities throughout the Twin Cities Metro area.

You will manage complex wounds, coordinate with interdisciplinary teams, and contribute to chronic disease management in post-acute settings. This full-time role offers base pay with productivity incentives and mileage reimbursement.

Qualifications

• Active Minnesota CNP license.
• Experience in wound care, geriatrics, primary care, post-acute care, or related settings preferred.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
• Strong clinical judgment and confidence managing medically complex patient populations.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Deliver on-site primary care and rounding services throughout senior living communities.
• Manage pressure injuries, diabetic wounds, post-surgical wounds, and complex chronic conditions.
• Coordinate care with physicians, nursing teams, therapists, caregivers, and facility leadership.
• Support preventative care initiatives, chronic disease management, and continuity of care.
• Help improve healing outcomes while reducing avoidable hospitalizations and complications.
