Job summary

Twin Cities Physicians is seeking a Wound Care Nurse Practitioner (AGNP/CNP) to provide on-site wound care across assisted living, memory care, and long-term care communities in Minnesota.

This full-time role offers $120,000 to $160,000+ total compensation (base salary plus productivity incentives) and mileage reimbursement, with full benefits, PTO, CME allowance, malpractice coverage, and DEA reimbursement.

Qualifications

• Active Minnesota Certified Nurse Practitioner license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility staff.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help reduce avoidable hospitalizations and improve patient outcomes.
