Job summary

Twin Cities Physicians seeks a Nurse Practitioner to deliver advanced wound care and community-based rounding inside assisted living and memory care settings across the Twin Cities Metro. This full-time role emphasizes specialized care delivery in senior living environments.

The successful candidate will have Minnesota NP licensure with AGNP preferred, strong clinical judgment, and the ability to travel. Competitive compensation with full benefits is offered.

Qualifications

• Active Minnesota NP license (CNP) with AGNP preferred.
• Experience in wound care, geriatrics, primary care, post-acute care, skilled nursing, or related settings.

Responsibilities

• Provide advanced wound care services and community-based rounding throughout senior living communities.
• Manage pressure injuries, diabetic wounds, post-surgical wounds, skin integrity concerns, and complex cases.
• Support chronic disease management, preventative care, and care coordination efforts.
• Collaborate with physicians, nurses, therapists, caregivers, and facility teams.
• Help improve outcomes while reducing avoidable hospitalizations and complications.
• Serve as a trusted clinical partner within the communities you support.
