Job summary

Twin Cities Physicians is seeking a Wound Care Nurse Practitioner to deliver on-site wound care across skilled facilities in the Twin Cities Metro area. The role focuses on complex wounds, chronic disease management, and post-acute medicine in geriatric settings.

Qualifications

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care, geriatrics, primary or post-acute care.
• Strong clinical judgment for medically complex patients.
• Valid driver's license and ability to travel across the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility staff.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help reduce avoidable hospitalizations and improve patient outcomes.
