Job summary

Twin Cities Physicians seeks an Advanced Wound Care Nurse Practitioner to serve senior living communities throughout the Twin Cities Metro. The role blends advanced wound care, community rounding, chronic disease management, and senior-focused medicine in a unique practice setting designed for on-site care at facilities.

You'll travel across multiple sites to deliver specialized care where patients live, with a compensation package that reflects base pay plus productivity incentives, mileage

Qualifications

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care and geriatrics.
• Strong clinical judgment and ability to manage complex patients.

Responsibilities

• Provide advanced wound care assessments and treatment plans.
• Conduct routine rounding visits across assisted living, memory care, and skilled nursing facilities.
• Manage medications, diagnostics, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Support chronic disease management and preventative care initiatives.
• Improve outcomes and reduce avoidable hospitalizations.
