Job summary

Twin Cities Physicians is seeking a Wound Care Nurse Practitioner (AGNP/CNP) to provide advanced wound care and rounding across post-acute and senior living communities in Minnesota. The role emphasizes improving healing rates, reducing complications, and strengthening care coordination.

We offer a competitive total compensation package ($120,000 to $160,000+), mileage reimbursement, and a comprehensive benefits package.

Qualifications

• Active Minnesota Certified Nurse Practitioner license (AGNP preferred)
• Experience in wound care, post-acute care, geriatrics, primary care, skilled nursing, or related settings
• Strong clinical judgment with medically complex patient populations
• Valid driver’s license and ability to travel throughout the Twin Cities Metro Area

Responsibilities

• Provide advanced wound care assessments and treatment plans for complex wounds
• Conduct rounding visits across post-acute, assisted living, memory care, and long-term care communities
• Manage pressure injuries, surgical wounds, diabetic ulcers, and skin integrity concerns
• Collaborate with physicians, nurses, therapists, and facility teams
