Job summary

Twin Cities Physicians is seeking a Wound Care Nurse Practitioner to provide advanced wound care assessments and management across post-acute and senior living sites in Minnesota. AGNP/CNP must hold active licensure and travel across the Twin Cities Metro.

You will work with physicians, nurses, and facility teams to reduce infection, manage complex wounds, and help lower hospital readmissions, while earning a competitive total compensation package including incentives and mileage reimbursement.

Qualifications

• Active Minnesota Certified Nurse Practitioner license (AGNP preferred).
• Experience in wound care, post-acute care, geriatrics, primary care, skilled nursing or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout post-acute, assisted living, memory care, and long-term care communities.
• Manage pressure injuries, surgical wounds, diabetic ulcers, skin integrity concerns, and related conditions.
• Collaborate closely with physicians, nurses, therapists, and facility teams.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help reduce avoidable hospital readmissions through proactive intervention and follow-up.
