Job summary

Twin Cities Physicians is seeking a Senior Living Nurse Practitioner to deliver advanced wound care and community-based rounding inside assisted living and memory care settings across Minnesota. This on-site role combines clinical autonomy with team-based care to improve outcomes for seniors.

You will manage pressure injuries and complex wounds, coordinate chronic disease care, and collaborate with physicians, nurses, therapists, and facility staff.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license; AGNP preferred.
• Experience in wound care, geriatrics, primary care, post-acute care, skilled nursing, or related settings.
• Strong clinical judgment and ability to practice independently.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care services and community-based rounding in senior living communities.
• Manage pressure injuries, diabetic wounds, post-surgical wounds, skin integrity concerns, and complex cases.
• Support chronic disease management, preventative care, and care coordination efforts.
• Collaborate with physicians, nurses, therapists, caregivers, and facility teams.
• Help improve outcomes while reducing avoidable hospitalizations and complications.
• Serve as a trusted clinical partner within the communities you support.
