The Association for Individual Development (AID) is seeking an experienced Case Manager Mental Health Professional (MHP) for the Community Support Team in Aurora, IL. The role focuses on mobile emergency crisis intervention and community-based support across hospitals, shelters, and client homes.

• Bachelor's degree in a human services field required.
• Experience in behavioral health preferred.
• Ability to work in community-based settings.

Responsibilities

• Attend to scheduled work hours and be flexible to meet client and program needs.
• Provide mobile emergency crisis intervention services and community support.
• Triage and delegate calls; provide crisis counseling and documentation on a timely basis.
• Coordinate care with CST team, hospitals, shelters, and client homes.
• Complete required assessments and maintain accurate records in cx360 per procedures.
