Job summary

Trinity Services, Inc. in Illinois is seeking a Certified Alcohol and Drug Counselor (CADC) and Mental Health Professional (MHP) to join the Crisis Care System.

The mobile crisis team provides on-site intervention, assessments, safety planning, case management, and links to community resources to support recovery and ongoing care. The role reports to the Program Director and operates in Homewood, Orland Park, Wilmington, and Joliet.

Qualifications

• CADC certification is required.
• Minimum one year of experience in mental health, crisis services, or related field.
• Valid driver's license, reliable transportation, and auto insurance required.
• Successful completion of all pre-employment screenings.

Responsibilities

• Provide on-site crisis intervention for behavioral health emergencies.
• Conduct comprehensive assessments and develop individualized crisis plans.
• Identify recovery goals and facilitate linkage to community services.
• Coordinate follow-up care and ensure continuity of care.
• Collaborate with internal and external providers and document accurately.
• Monitor high-risk factors and obtain QMHP consultation when needed.
• Participate in trainings and team meetings.
• Maintain timely, accurate service documentation per agency standards.
