One Hope United in Illinois seeks a Mobile Crisis Case Manager to deliver crisis intervention, case management, and counseling in community settings under supervision. You will stabilize mental health symptoms, link clients to services, and provide in-home assessments to support families and youth.

The role involves daytime crisis coverage with occasional evenings, weekends, and holidays, and requires coordination with Medicaid-funded services and documentation compliance.

Competenze

• BS/BA degree; BSW preferred in a human service-related field
• 3 or more years related experience in child welfare
• Valid driver’s license and acceptable driving record with OHU required levels of coverage for auto insurance

Mansioni

• Daytime crisis intervention services, hours starting at 8 am-5 pm
• Some coverage of nights, weekends, and holidays as needed
• Conduct Crisis/Safety Assessments with ETA 90 minutes
• Link clients to health insurance, therapy, housing resources, etc.
• Provide in-home services to assess safety and needs
• Follow ARC practice framework in services
• Complete Medicaid-compliant documentation
• Represent the agency at court hearings as necessary
