Job summary

One Hope United is seeking a Mobile Crisis Case Manager to deliver community-based crisis intervention, case management, and mental health services for children and adults in Illinois. The role emphasizes rapid response, resource linkage, and coordinated care, with follow-up after crisis to ensure safety and stability.

The position requires a BS/BA degree (BSW preferred), at least 3+ years in child welfare, a valid driver’s license, and strong organizational and communication skills.

Qualifications

• BS/BA degree; BSW preferred in a human service-related field.
• 3+ years related experience in child welfare.
• Valid driver’s license and acceptable driving record with OHU required levels of coverage for auto insurance.

Responsibilities

• Provide daytime crisis intervention services from 8 am to 5 pm with potential after-hours coverage.
• Respond to crisis with an ETA of 90 minutes and continue until the crisis is resolved.
• Link clients to health insurance, therapy, housing resources, medical treatment, and other supports.
• Conduct in-home assessments, safety planning, and service provision per ARC framework.
• Maintain Medicaid-compliant documentation and complete required paperwork timely.
• Coordinate with court systems as needed and provide transportation within guidelines.
