Job summary

The Association for Individual Development (AID) is seeking a Case Manager QMHP to join our Community Support Team in Elgin, IL. This full-time position requires a Master's degree in a human services field and provides direct services to individuals with mental health needs.

Responsibilities include crisis counseling, client assessments, and collaborative treatment planning, with a focus on community integration and documentation accuracy. Benefits include paid time off and ongoing training.

Qualifications

• Master's degree in human services field required.
• At least one year experience working with individuals with mental illness; knowledge of DSM V-TR or current edition & Rule 132.
• Ability to provide services in clients’ homes and in the community.
• Valid Illinois Driver's license and good driving record.

Responsibilities

• Attend to scheduled work hours and be flexible to meet client and program needs.
• Respond to calls from shift start to shift end.
• Provide independent crisis counseling and develop crisis/treatment plans with client and CST.
• Maintain documentation and billing records in cx360.
• Promote awareness of mental health and collaborate with CST team.
