Job summary

The Association for Individual Development (AID) is seeking a Case Manager Mental Health Professional (MHP) within the Community Support Team in Aurora, IL. The role focuses on mobile crisis intervention and community engagement to help clients achieve stable, healthy lives.

Responsibilities include conducting assessments, crisis counseling, documentation, and collaboration with providers, with an emphasis on off-site client interactions and adherence to program standards.

Qualifications

• Bachelor's degree in human services field required.
• Masters degree preferred.
• At least one year experience working with individuals with mental illness.
• Knowledge of DSM V-TR or most current edition & Rule132.

Responsibilities

• Provide mobile emergency crisis intervention services and community support and engagement services.
• Engage with clients in homes, shelters, hospitals and other community settings.
• Complete required documentation on a timely basis including billing notes and assessments.
• Attend clinical supervision monthly and participate in CST meetings.
• Maintain certifications in First Aid, CPR, CPI and related trainings.
