Job summary

Association for Individual Development in Illinois is seeking a Street Outreach Case Manager to contact individuals misusing Crisis Services and engage them in clinically appropriate services and shelter/housing. The role involves outreach, case management, advocacy, and warm hand-offs in community settings, with hours including evenings and weekends.

Requirements include a Bachelor's degree in human services and experience with at-risk populations; ASSIST/SAMHSA SOAR certification is preferred,

Qualifications

• Bachelor’s degree in human services field required.
• Masters degree preferred.

Responsibilities

• Attend to scheduled work hours and be flexible to meet client and program needs.
• Respond to outreach calls from shift start to shift end.
• Provide outreach and case management, advocacy, and support services to individuals with mental health diagnoses.
• Complete required documentation (assessments, treatment plans, case notes, ROI) on a timely basis.
• Promote client rights and assist with entitlements.
