Ibis Healthcare in Hillsborough County, FL, seeks a Care Manager Dispatcher for the Adult Crisis Mobile Response Team. You will provide direct care, coordinate resources, and plan care for individuals and families in crisis, traveling to client homes in the community.

Join a 60-bed crisis services environment with a focus on immediate support and evidence-based care delivery in a variety of neighborhoods.

Formación

• Must have a Bachelor's Degree in Human Services or related field.
• Must be able to lift and carry up to 50lbs of dead weight.
• Must have excellent interpersonal skills.
• Valid Florida Driver's license, reliable transportation, and insurance.

Responsabilidades

• Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
• Complete all clinical and administrative documentation based on established timelines as per program requirements.
• Communicate effective and maintain cooperative relationships with internal and external resources.
• Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements.
• Maintain efficient workflow management and organization as per program requirements.
• Maintain proficient quality of clinical documentation as per program requirements.
• Other duties as assigned or needed as per program requirements.
