Job summary

Ibis Healthcare is seeking a Care Manager for the Adult Crisis Stabilization Unit to advocate, coordinate, and ensure continuity of evidence-based care for individuals and families. You will work within a multidisciplinary treatment team, performing triage, assessment, planning, and effective communication with diverse stakeholders.

The role emphasizes completing assessments, developing care and aftercare plans, and maintaining timely documentation in a fast-paced setting.

Qualifications

• Bachelor's degree in a Human Services related discipline and at least 1 year experience in the mental health field.
• Ability to read, analyze, and interpret clinical records, procedures, or governmental regulations.
• Ability to write reports and document clinical activities and effectively communicate with managers, patients, service providers, and other stakeholders.

Responsibilities

• Complete thorough assessments and coordinate community resources based on patient needs.
• Complete all clinical and administrative documentation within established timelines.
• Communicate effectively and maintain cooperative relationships with internal and external resources.
• Develop service plans, treatment plans and/or aftercare plans with clear goals and objectives.
