E3applicants is seeking a Care Manager to join its Mobile Crisis Response Team. The role provides direct care to individuals with behavioral health needs and coordinates evidence-based care in home settings within Hillsborough County, using both personal and agency vehicles.

The position requires a Bachelor's Degree in Human Services or a related field, strong interpersonal skills, and the ability to lift up to 50 lbs. A Florida driver's license and reliable transportation are required.

Qualifications

Responsibilities

• Complete thorough assessments and coordinate community resources based on patient needs as per program requirements.
• Complete all clinical and administrative documentation on established timelines.
• Communicate effectively and maintain cooperative relationships with internal and external resources.
• Develop service plans, treatment plans, and aftercare plans with clear goals.
• Maintain efficient workflow management and organize according to program requirements.
• Maintain quality of clinical documentation as per program requirements.
• Other duties as assigned as per program requirements.

Job description

A Care Manager for our Mobile Crisis Response Team willprovide direct care to individuals served with behavioral health needs, including supporting individuals served with coordination and continuity of evidence-based care for individuals and families in need through a collaborative triage, assessment, planning, and communication process. The position requires traveling within Hillsborough County in a personal vehicle or agency vehicle to facilitate in-home client services in various socioeconomic neighborhoods.

JOB REQUIREMENTS:

• Must have a Bachelor's Degree in Human Services or a related field of study in Human Services,Behavioral Health, or Psychology.
• Must be able to lift and carry up to 50lbs of dead weight
• Must have excellent interpersonal skills

Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe, supportive environment.

This is a 60-bed Adult Crisis inpatient care facility.

Office setting within the Crisis Center ?

Must be capable of lifting 50 pounds of dead weight alone.
