Job summary

Ibis Healthcare seeks a Care Manager for Central Intake in Tampa to advocate, coordinate, and ensure continuity of care for individuals and families in crisis. You will participate in triage, assessment, planning, and communication within a multidisciplinary team in a 24/7 Adult Crisis Stabilization setting.

Qualifications

• Bachelor's degree in a Human Services-related discipline and at least 1 year of experience in the mental health field.
• Ability to read, analyze, and interpret clinical records, procedures, or governmental regulations.
• Ability to write reports and document clinical activities and effectively communicate with managers, patients, service providers, families, and other stakeholders.

Responsibilities

• Complete thorough assessments and coordinate community resources based on patient needs.
• Complete clinical and administrative documentation within established timelines.
• Communicate effectively and maintain cooperative relationships with internal and external resources.
• Develop service plans, treatment plans, and aftercare plans with clear goals.
• Maintain efficient workflow management and organization as per program requirements.
• Maintain high quality of clinical documentation as per program requirements.
• Other duties as assigned or needed per program requirements.
