Qualifikationen

• Bachelor's degree in Behavioral Sciences required (or a nonrelated field with 6 years' comparable experience, including 1 year with SPMI/addictive disease population).
• Minimum of 3 years of experience, including 1 year with the SPMI/addictive disease population, preferably in full-time status.
• Basic Life Support (BLS) Certification from the American Heart Association is required.
• Preferred certifications: CTRS, CRC, or CPRP from respective certifying bodies.

• Provides strong case management support including psychosocial needs assessments, care coordination, discharge planning, and linkage to community-based behavioral health resources.
• Collaborates with licensed clinicians, nursing staff, and interdisciplinary teams to ensure continuity of care and follow-up for high-risk patients.
• Utilizes knowledge of Atlanta metro community resources to facilitate appropriate referrals to housing programs, outpatient behavioral health providers, crisis services, and social support agencies.
• Supports patients with complex psychosocial needs such as homelessness, co-occurring disorders, and limited support systems.
• Documents patient interactions and resource coordination activities accurately in the electronic medical record.
