Job summary

Qualifications

• Bachelor's degree in Behavioral Sciences or related field required; six years experience in lieu of degree.
• Minimum of three years experience with SPMI and/or Addictive Disease population, preferably full-time.
• Basic Life Support (BLS) certification required.
• Certification in CTRS, CRC, CPRP preferred.

Responsibilities

• Provides strong case management, including psychosocial needs assessments and discharge planning.
• Coordinate care and linkage to community-based behavioral health resources.
• Collaborate with licensed clinicians and interdisciplinary teams to ensure continuity of care.
• Document patient interactions and resource coordination in the EMR.
