Qualifications

• Bachelor's degree in Behavioral Sciences or related field is required or equivalent experience with six years in related roles.
• Minimum of three years of experience, with one year in SPMI/addictive disease population, preferably full-time.
• BLS Certification from American Heart Association required.

Responsibilities

• Provides strong case management support, including psychosocial needs assessments, care coordination, discharge planning, and linkage to community-based resources.
• Collaborates with licensed clinicians, nursing, and interdisciplinary teams to ensure continuity of care for high-risk patients.
• Uses knowledge of Atlanta metro community resources to facilitate referrals to housing programs, outpatient behavioral health providers, crisis services, and social support agencies.
• Supports patients with complex psychosocial needs, including homelessness and co-occurring disorders.
• Documents patient interactions and resource coordination activities accurately in the EMR.
