Job summary

Grady Health System in Atlanta is seeking a Mental Health Technician II to provide group and individual training, crisis prevention, and care coordination across hospital, outpatient clinic, or community settings. The role includes linking patients to community resources and supporting care teams.

You will work with clinicians to support individuals with serious mental illness or addictive disease, assist with discharge planning, and help coordinate services across multiple settings.

Qualifications

• Bachelor's degree in Behavioral Sciences or related field required; nonrelated degree with 6 years of relevant experience also acceptable.
• Minimum of 3 years of experience, with 1 year in SPMI or addictive disease populations.
• BLS certification from American Heart Association required.
• Community experience preferred.
• Certification in CTRS, CRC or CPRP preferred.

Responsibilities

• Provide case management support, including psychosocial needs assessments and discharge planning.
• Coordinate care and linkages to community-based behavioral health and social services.
• Collaborate with clinicians and nursing to ensure continuity of care for high-risk patients.
• Leverage Atlanta resources to facilitate referrals to housing, crisis services, and support agencies.
• Support patients with complex psychosocial needs, including homelessness and limited support systems.
