Job summary

Grady Health System is seeking a Mental Health Technician II to provide services to individuals experiencing serious mental illness, including group and individual training on social and coping skills, and linkage to community resources.

The role supports crisis prevention, coordination of care, and referrals across hospital, outpatient, and community settings. Strong collaboration with clinicians and interdisciplinary teams is essential.

Qualifications

• Bachelor's degree in Behavioral Sciences required; or bachelor's in nonrelated field with six years of relevant experience.
• Minimum three years of experience, with one year in SPMI/Addictive Disease population.
• Basic Life Support (BLS) Certification from American Heart Association required.
• Community experience preferred.
• Certification in CTRS, CRC, or CPRP preferred.

Responsibilities

• Provide strong case management support, including psychosocial needs assessments, care coordination, discharge planning, and linkage to community-based resources.
• Collaborate with licensed clinicians, nursing staff, and interdisciplinary teams to ensure continuity of care and follow-up for high-risk patients.
• Utilize knowledge of Atlanta metro community resources to facilitate referrals to housing, outpatient behavioral health, crisis services, and social supports.
• Support patients with homelessness, co-occurring disorders, and limited support systems.
• Document patient interactions and resource coordination activities accurately in the EMR.
