Job summary

Grady Health System is seeking a Mental Health Technician II to provide services to individuals with serious mental illness. The role includes group and individual training on social and coping skills, care coordination, and linkage to community services in hospital, outpatient, or home/clinic settings.

The ideal candidate will have a behavioral sciences degree, at least 3 years of relevant experience (1 year with SPMI/addictive disease), and BLS certification.

Qualifications

• Bachelor's degree required; alternative: bachelor's degree in nonrelated field with 6 years relevant experience, including 1 year with SPMI/addictive disease population.
• Minimum of three (3) years of experience with one (1) year in SPMI/addictive disease population, preferably full-time.
• BLS Certification from American Heart Association is required.
• Prefer certifications: CTRS, CRC, or CPRP from respective boards.

Responsibilities

• Provides strong case management support including psychosocial needs assessments, care coordination, discharge planning, and linkage to community resources.
• Collaborates with licensed clinicians, nursing staff, and interdisciplinary teams to ensure continuity of care for high-risk patients.
• Utilizes knowledge of Atlanta metro resources to facilitate referrals to housing programs, behavioral health providers, crisis services, and social support agencies.
• Supports patients with complex psychosocial needs, including homelessness and limited support systems.
• Documents patient interactions and resource coordination accurately in the EMR.
