Qualifications

• Bachelor's degree in Behavioral Sciences, including Criminal Justice, Cultural Studies, Anthropology, or Sociology is required. In lieu of degree in Behavioral Science, will accept bachelor's degree in nonrelated field, with six years of comparable job experience, with one year of experience with the SPMI and/or Addictive Disease population, preferably in a full-time status.

Responsibilities

• Provides strong case management support, including psychosocial needs assessments, care coordination, discharge planning, and linkage to community-based behavioral health and social service resources.
• Collaborates with licensed clinicians, nursing staff, and interdisciplinary teams to ensure continuity of care and follow-up for high-risk patients.
• Utilizes knowledge of Atlanta metro community resources to facilitate appropriate referrals and supports patients with complex psychosocial needs.
