Job summary

EMPACT-SPC/La Frontera Arizona seeks an experienced Case Manager (General Mental Health) to deliver in‑home, in‑office and community services, including crisis intervention and transportation, for adults, children, and families in Maricopa County, AZ. The role follows the Arizona Practice Model and 12 Principles.

The ideal candidate has a BHT with relevant experience or an equivalent combination and must possess a valid AZ driver license, fingerprint clearance, and reliable transportation.

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Flexible work hours and ability to meet with individuals in home, community or office.
• Finger Print clearance card or eligibility to obtain one.
• Must have a valid AZ driver's license with no major infractions in the past three years.
• Must have regular access to reliable transportation.
• Bi-lingual (Spanish) ability preferred. Spanish language differential available.
• Must be at least 21 years of age

Responsibilities

• Provide direct in‑home, in‑office and community case management and crisis intervention.
• Coordinate Child and Family Team Meetings/Adult Recovery Team Meetings and document outcomes.
• Deliver at least 30 hours of direct client contact per week within productivity standards.
• Develop and maintain effective working relationships with related state, community mental health, and contracting organizations.
• Maintain current client records and ensure compliant documentation.
• If driving, maintain valid AZ driver license and coverage; maintain clean driving record.
• Attend regular weekly supervision and seek additional supervision as needed.
