La Frontera EMPACT - Suicide Prevention Center is seeking an experienced Case Manager (General Mental Health) to deliver case management, crisis intervention, transportation, and education to adults, children, and families. You will coordinate services per the Arizona Practice Model and the 12 Principles, aiming for strong community-based outcomes.

Qualifications

Must meet behavioral health technician (BHT) requirements with a degree level and experience option.
Flexible work hours and ability to meet clients in home, community, or office.
Fingerprint clearance card required or eligible to obtain one.
AZ driver’s license with clean record in last 3 years.
Regular access to reliable transportation.
Bilingual (Spanish) ability preferred.

Responsibilities

Provide case management and coordination services including direct service and crisis intervention.
Arrange transportation and education to clients and families.
Coordinate services per the Arizona Practice Model and 12 Principles.
Schedule and participate in Family/Recovery Team meetings and document outcomes.
Maintain client records in compliance with RBHA, state and federal requirements.
