Job summary

EMPACT-SPC/La Frontera Arizona is seeking an experienced Case Manager (General Mental Health) to join a dedicated clinical team in San Tan Valley. You will provide case management, crisis intervention, transportation, and education to adults, children, and families, coordinating services under the Arizona Practice Model and 12 Principles.

The role requires a bachelor's degree in a behavioral health field, flexibility to meet clients in home or community settings, valid AZ driver’s license,

Qualifications

• Bachelor's degree in a behavioral health-related field or equivalent combination of education/experience meets BHT requirements per AZDHS/BHS.
• Bi-lingual (Spanish) ability preferred.

Responsibilities

• Provide case management and coordination services including direct service, crisis intervention, transportation, and education to adults, children, and families.
• Coordinate services in line with the Arizona Practice Model and the 12 Principles.
• Maintain client records in current status per agency, RBHA, state and federal requirements.
• Attend supervision and meet productivity expectations.
