Job summary

La Frontera EMPACT - Suicide Prevention Center in Arizona is hiring High Needs Case Managers to provide direct crisis intervention, in-home and community services, and intensive case management for families and children.

You will coordinate team meetings, document outcomes, and work toward functional outcomes while maintaining flexible hours including evenings/weekends as needed. Prior experience with children and knowledge of Arizona 12 Principles are valued.

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Behavioral health experience is required.
• Previous experience working with children is preferred.
• Knowledge of the CFT process and Arizona 12 Principles.
• Requires the ability to be flexible and problem-solve and to have the ability to use creativity in the approach to service delivery.
• Must have a Fingerprint clearance card or be eligible to obtain one.
• Must have a valid AZ driver's license with no major infractions in the past three years.
• Must have regular access to reliable transportation.
• Bi-lingual (Spanish) a plus. Language differential available.
• Must be at least 21 years of age at the time of employment.

Responsibilities

• Provide direct in-home, in-office, in-school, and community services including crisis intervention, transportation, stabilization, client supervision, parent training and child behavior management.
• Facilitate, schedule, and provide follow-up on family, youth, and adult team meetings (ACT or CFT) and document outcomes.
• Deliver at least 40 hours of work per week with a target of 30 hours of direct client contact; maintain monthly/annual clinical documentation.
• Coordinate with Clinical Coordinator/Director on communication with other departments and external stakeholders.
• Develop and maintain effective relationships with related states, community mental health and contracting organizations.
• Attend meetings, trainings, and agency events outside regular hours upon request.
