Job summary

EMPACT-SPC/La Frontera Arizona seeks a High Needs Case Manager to support children and families with high-intensity services across in-home, in-school, and community settings. You will coordinate crisis intervention, transportation, and parenting education while maintaining rigorous documentation and adherence to Arizona Principles.

The role requires BH experience, a relevant degree, and flexibility for evenings or weekends.

Qualifications

• Must meet BHT requirements: BA or higher + 1 year BH experience; OR Associate + 2 years; OR HS diploma + 4 years BH experience.
• Behavioral health experience is required.
• Previous experience working with children is preferred.
• Knowledge of the CFT process and Arizona 12 Principles.
• Requires flexibility and problem-solving and creative service delivery.
• Must have Fingerprint clearance card or be eligible.
• Must have valid AZ driver's license with no major infractions in 3 years.
• Must have regular access to reliable transportation.
• Bi-lingual (Spanish).
• Must be at least 21 years of age.

Responsibilities

• Provide direct in-home, in-office, in-school, and community services including crisis intervention and transportation.
• Facilitate and document ACT/CFT meetings; coordinate referrals and natural supports.
• Deliver 40 hours/week with 30 hours direct client contact; maintain clinical documentation monthly/annually.
• Coordinate with Clinical Coordinator/Director and other departments.
• Develop and maintain relationships with states and community partners; ensure continuity of care.
