Job summary

La Frontera Center Inc. is seeking an experienced Case Manager (General Mental Health) to join EMPACT-SPC in San Tan Valley, AZ.

You will provide case management and coordination services, including direct service, crisis intervention, transportation, and education to adults, children, and families. You will work within the Arizona Practice Model and 12 Principles, maintain thorough documentation, collaborate with partners, and meet weekly supervision.

Qualifications

• Bachelor's degree in behavioral health or equivalent education/experience meeting AZDHS/BHS requirements.
• Fingerprint clearance card or eligibility to obtain one.
• Valid AZ driver’s license with clean driving record (past 3 years).
• Regular access to reliable transportation.
• Bi-lingual (Spanish) ability preferred.

Responsibilities

• Provide direct in-home, in-office and community case management and coordination services.
• Facilitate meetings and coordinate services per the Arizona Practice Model and 12 Principles.
• Maintain current, thorough documentation of services and client progress.
• Coordinate with families, youth, and adults, and engage natural supports.
• Attend supervision and meet productivity goals as required.
