Job summary

EMPACT-SPC, part of La Frontera Arizona, is seeking a High Needs Case Manager to work with children and families requiring intensive services. You will provide direct crisis intervention, in-home and school-based support, transportation, and coordination of treatment for a caseload of about 15 families.

The role requires BHT with a BA/BS or higher or an AA or high school with experience; bilingual Spanish is preferred; valid AZ driver license; fingerprint clearance; and flexibility including

Qualifications

• Must meet BHT requirements with a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Behavioral health experience is required.
• Previous experience working with children is preferred.
• Knowledge of the CFT process and Arizona 12 Principles.
• Fingerprint clearance card or eligible to obtain one.
• Valid AZ driver’s license with no major infractions in 3 years.
• Regular access to reliable transportation.

Responsibilities

• Provide direct in-home, in-office, in-school, and community services including crisis intervention and transportation.
• Schedule and conduct family, youth, and adult team meetings; document and share outcomes with the team.
• Maintain clinical documentation and meet timelines; flexible hours including evenings/weekends as needed.
• Coordinate with family to deliver services aligned to the 12 Arizona Principles.
• Develop and maintain relationships with related agencies and partners.
